Here at Emslie’s Floreat Pharmacy we are aware that over 50% of people over the age of 50 years are taking several medications daily, and a high percentage of these people fail to take their medication as prescribed.

MedsCheck At Emslie’s Floreat Pharmacy

A MedsCheck and Diabetes MedsCheck is a service funded by the Australian Government and involves one-on-one time with your Pharmacist to discuss all your medicines.

There is no cost to the consumer to receive either the MedsCheck or Diabetes MedsCheck services.

Together we can ensure you are getting optimal benefits from your medicines.

A MedsCheck or Diabetes MedsCheck will help you better manage your medicines by increasing your knowledge about your medicines, giving you confidence in taking them and therefore reducing the risk of any adverse mishaps.

To be eligible for a MedsCheck you need to:

• Be a Medicare and/or Department of Veteran’s Affairs cardholder
• Have not received a MedsCheck, Diabetes MedsCheck, Home Medicines Review (HMR) or Residential Medication Management Review (RMMR) in the past 12 months
• Be living at home in a community setting
• Be taking five or more prescription medicines
• Have had a recent significant medical event

To be eligible for a Diabetes MedsCheck you need to:

• Be a Medicare and/or DVA cardholder
• Have not received a MedsCheck, Diabetes MedsCheck, Home Medicines Review (HMR) or Residential Medication Management Review (RMMR) in the last 12 months
• Be living at home in a community setting
• Have recently been diagnosed with type 2 diabetes (in the last 12 months) or have less than ideally controlled type 2 diabetes
• Be unable to gain timely access to existing diabetes education /health services in their community.
