A Home Medicines review involves your GP and an accredited pharmacist of your choice.
Your GP will write a referral to your pharmacist.
Your pharmacist will arrange to conduct an interview with you and then send a report back to the GP.
Any recommendations or changes to your medication regime will be discussed with you by your GP.

An explanation of why and when to take the medicines.
