If you feel you could benefit from an HMR, your GP can set up the review by writing a letter of referral. You can then take this letter to Emslie’s Pharmacy, and book a time and date with our team that suits you.
A home medication review pharmacist will then visit you in the comfort of your home and go through your medicines carefully with you. Your pharmacist will then write a report back to your GP, who will then discuss any findings and recommendations with you.
