Each year, more than 140,000 Australians are admitted to hospital with problems caused by their medicine. This could be taking the wrong dosage, mixing and matching the wrong medicines, or taking a medicine that wasn’t prescribed.

What’s involved in a Home Medicines Review (HMR)?

Home medication review (HMR) involves your GP and a compounding pharmacy (compounding chemist) like Emslie’s Pharmacy. If you feel you could benefit from an HMR, your GP can set up the review by writing a letter of referral. You can then take this letter to Emslie’s Pharmacy, and book a time and date with our team that suits you.

A home medication review pharmacist will then visit you in the comfort of your home and go through your medicines carefully with you. Your pharmacist will then write a report back to your GP, who will then discuss any findings and recommendations with you.

A home medication review chemist visit, review and report is paid for by the Australian Government. You won’t have any out-of-pocket costs for the HMR home visit.
