Home Medication Reviews

When was the last time you had a good look through your medicine cabinet? Chances are, you may have a few outdated or even unknown medicines floating around from years past. If this sounds like you, you’re not alone!

Each year, more than 140,000 Australians are admitted to hospital with problems caused by their medicine. This could be taking the wrong dosage, mixing and matching the wrong medicines, or taking a medicine that wasn’t prescribed.

It has been shown that up to 69% of hospitalisation cases due to home medication misuse could have been avoided. Older people are particularly at risk. To help you stay safe at home and use the medication that’s right for you, Emslie’s Floreat Pharmacy offer a comprehensive Home Medication Review program.

What’s involved in a Home Medicines Review (HMR)?

Home medication review (HMR) involves your GP and a compounding pharmacy (compounding chemist) like Emslie’s Pharmacy. If you feel you could benefit from an HMR, your GP can set up the review by writing a letter of referral. You can then take this letter to Emslie’s Pharmacy, and book a time and date with our team that suits you.

A home medication review pharmacist will then visit you in the comfort of your home and go through your medicines carefully with you. Your pharmacist will then write a report back to your GP, who will then discuss any findings and recommendations with you.

What are some of the benefits of Home Medicines Review (HMR)?

By taking the time to speak to an experienced pharmacist, you can expect a variety of benefits from your home medication review (HMR) visit. Some of these include:

How much does an Home Medication Review (HMR) cost?

A home medication review chemist visit, review and report is paid for by the Australian Government. You won’t have any out-of-pocket costs for the HMR home visit. However, your GP may charge you a consultancy rate to receive your results, or they may bulk-bill you. Talk to your GP to see what options are available to you.
